Poland are out of the World Cup after a 3-0 win by Colombia.

Radamel Falcao and Juan Fernando Quintero had chances to double their lead but blazed both opportunities over the bar.

A ball over the top fell to Robert Lewandowski but David Ospina saved with his body to deny the Bayern Munich man.
